Patient Registration Information & Forms
New Patients
It’s our office policy that new patients must arrive 30 minutes prior to your scheduled appointment. Your appointment will need to be rescheduled if you haven’t arrived at least 15 minutes prior to your appointment time. This will allow adequate time for registration and form completion.
Current Patients
Established patients should arrive on time and may be asked to reschedule if more than 10 minutes late.
Please bring the following:
- Photo ID
- List of Current Medications
- Insurance Card
- New Patient Information Packet Completely Filled Out
(Optional)
If you are unable to keep your appointment, please contact us at 757 481 4422 within 24 hours of your scheduled appointment.
Skin Check Appointments
If coming for a skin check please avoid make up, nail polish, intricate sprayed hair styles that may hide potentially concerning lesions. If unable to avoid the above, please be prepared to remove them before your exam.
Our Insurance Policy
All insurance copayments are due at the time of service.
Referrals
If your insurance requires a referral to see a specialist, please request it from your primary care provider and allow sufficient time for processing before your appointment. Most providers require 48 hours notice for referrals.

Minors
Please remember that patients under the age of 18 must be accompanied by their parent or guardian at their appointment.
